Approved uses

Miosis during intraocular surgery

No more than half a millilitre instilled intraocularly into the anterior chamber, before or after securing sutures

Intraocular pressure elevation in the first 24 hours after cataract surgery

No more than half a millilitre instilled intraocularly

Blunts the early post-operative pressure spike, which is the reason it is used beyond producing miosis

Contraindications

Should not be used in those persons showing hypersensitivity to any of the components of this preparation.

Available Forms

0.01% carbachol intraocular solution, 1.5 mL fill in a 2.0 mL glass vial, twelve to a carton.

Mechanism of Action

Carbachol is a potent cholinergic (parasympathomimetic) agent that constricts the iris and ciliary body, producing miosis and a reduction in intraocular pressure.

For single-dose intraocular use only; discard the unused portion.

Use with caution in acute cardiac failure, bronchial asthma, peptic ulcer, hyperthyroidism, gastrointestinal spasm, urinary tract obstruction and Parkinson's disease.

Common Side Effects

Ocular: Corneal clouding, persistent bullous keratopathy, retinal detachment and postoperative iritis following cataract extraction have been reported. Systemic: Side effects such as flushing, sweating, epigastric distress, abdominal cramps, tightness in urinary bladder, and headache have been reported with topical or systemic application of carbachol.

Pregnancy & Lactation

Pregnancy: No adequate and well-controlled studies in pregnant women; use during pregnancy only if the potential benefit justifies the potential risk to the fetus.

Lactation: Not stated on the FDA label.

Pediatric Use

Safety and efficacy in pediatric patients have not been established.
